2010-2011 High School Sports Physicals
Presented by
Orthopaedic Associates of Wisconsin will be conducting Sports Physicals for all High School athletes required to fulfill WIAA regulations. The dates, times, and locations are listed below. There is a $15.00 charge for each athlete and the funds collected will be donated back to each individual high school athletic program. Each athlete is required to fill out a medical history form prior to the physical and bring it with them. Forms and additional information are available at the Athletics Office.
