Steps to Take to Participate in Athletics: These items must be complete to participate on the first day of the season. Please take care of these items at least 2 weeks prior to the start of your season to allow sufficient time for processing to avoid delays in your athlete’s eligibility to participate.
- Sign up on the google form on your particular sports home page on the AHS website (www.arrowheadschools.org/activities/sports). The link to the form is on the righthand side.
- Confirm a current physical examination (green) card is on file in the Activities Office. Per the WIAA, the physical examination (green) card is valid for the following two SCHOOL years if it is obtained after April 1 of that school year. Otherwise, a physical exam taken before April 1 of that school year is valid for the remainder of that school year and the following school year.
- Physical exam cards are available in the North Campus Activities Office or the South Campus front office. We need the actual card to be signed and dated. Unfortunately, we cannot send this form electronically.
- Please obtain a doctor’s signature and date of the physical exam. A parent / guardian must also sign and date the green card.
- Please return the green card to the North Campus Activities Office.
- If the doctor is unable to sign the actual green card, a physical examination letter from the doctor’s office with the physician’s signature and date of physical exam will be accepted by the Activities Office BUT we still require a parent / guardian to sign and date the actual green card.
- Physician offices may fax their exam letter to our office at 262-367-1870.

Sign three online forms found on your Arrowhead Family Access Skyward account. The athlete and a parent/guardian must sign the three forms annually. We can automatically see the forms when complete so there is no need to print them. These forms are available in July for the upcoming school year.
The forms are:
* Annual Athletic Permission Form
* Annual Concussion/Cardiac Arrest Form
* Annual WIAA High School Athletic Eligibility Information Form

Pay an annual $195 district athletic participation fee. This fee can be paid online through your Family Access Skyward account or by cash / check made payable to AHS and turned into the Activities Office or Jen Justice in the North Campus front office. Fees should be paid by the start of your season or immediately after making the team for a cut sport.
